COMT Gene: How It Shapes Your Stress Response and Oestrogen Clearance
The gene that decides how you handle stress, and your own oestrogen
COMT is an enzyme that clears dopamine and adrenaline from your system after stress, and also helps clear used oestrogen. A slower version can mean stress lingers longer than it seems to for other people, and PMS or hormone-related symptoms can feel disproportionate. A faster version tends to mean quick recovery from stress, but a need for more stimulation to feel motivated. Neither is a diagnosis, it’s a piece of context for understanding a pattern you may already recognise in yourself.
“I fall apart under pressure” and “I thrive under pressure” tend to get treated as personality traits, something fixed about who you are. Often, though, part of the answer sits somewhere more physical than that. COMT is one of the genes with the biggest influence on how a stress response plays out in the body, and, less commonly discussed, on how efficiently the body clears its own oestrogen. Looking at one without the other only tells half the story.
What This Enzyme Is Actually Doing
COMT stands for Catechol-O-Methyltransferase. Think of it as a clean-up crew: once dopamine and adrenaline have done their job during a stressful moment, or once oestrogen has been used and needs to move on, COMT is part of what clears them out of the system.
How quickly that clean-up happens depends on which COMT variant you carry. A faster-clearing version means dopamine and adrenaline are broken down quickly, which often shows up as recovering from stress rapidly, but sometimes also as needing more stimulation before you feel motivated or genuinely switched on. A slower-clearing version leaves those same chemicals circulating for longer. That can translate into sharper focus during a short burst of pressure, alongside a nervous system that finds it harder to power back down afterwards: overthinking into the evening, feeling wired but exhausted, or noticing that a stressful event seems to stay in your system well after it’s actually over.
Most people sit somewhere between these two patterns rather than firmly at either end. But recognising which direction you lean can explain a surprising amount about how you experience pressure, caffeine, sleep, and even certain supplements.

Most people sit somewhere between these two columns rather than firmly in one, which is worth keeping in mind when reading down the list.
COMT doesn’t tell you whether you’re “good” or “bad” under stress. It tells you how long the stress response lingers once the moment itself has passed.
Where Oestrogen Enters the Picture
The same enzyme responsible for clearing dopamine and adrenaline is also involved in one of the pathways the body uses to process used oestrogen. COMT is one of the enzymes involved in this process. This is the connection that rarely gets mentioned alongside the stress side of COMT, even though for many people it’s the more noticeable one.
A slower COMT enzyme can mean oestrogen metabolites clear more gradually as well. That doesn’t necessarily mean oestrogen levels themselves are high, it means the byproducts of processing oestrogen may hang around a little longer than they would for someone with faster COMT activity. For some people, that shows up as heavier or more prolonged PMS-type symptoms, and it’s part of what sits behind the idea of oestrogen dominance .
This is also why a standard blood test can come back showing a perfectly normal oestrogen level, even when oestrogen-related symptoms are very much present.
A blood test measures how much oestrogen is circulating at that moment, not what happens to it afterwards.
COMT works downstream of that measurement, on the metabolites left over once oestrogen has already been used. Slower clearance at that stage doesn’t necessarily raise the oestrogen level itself, so it isn’t something a standard blood panel is designed to pick up. What This Means If You’re on the Pill or Taking HRT
The same clearance pathway applies whether the oestrogen your body is processing was made internally or introduced through the oral contraceptive pill or menopausal hormone replacement therapy.
If COMT activity is slower, the synthetic or bioidentical oestrogen from these sources may also be cleared more gradually, alongside its own metabolites. For some people, this shows up as a stronger reaction to starting a new pill formulation, more noticeable side effects with hormone therapy, or a sense that the same dose seems to affect them more than it appears to affect other people on an identical prescription.
It doesn’t mean the pill or HRT is the wrong choice, and it isn’t a reason to avoid either. It’s more that COMT activity is one of several factors shaping how a given dose is experienced, alongside liver function, gut health and individual metabolism, which is worth understanding before assuming a dose or formulation simply isn’t suited to you.
And What About Xenoestrogens?
Oestrogen exposure isn’t limited to what the body produces or what’s prescribed. Xenoestrogens are another source the same clearance pathway has to deal with, and COMT plays a role here too. Once these compounds are processed by the body, some of the same detoxification steps that clear internal and prescribed oestrogen are called on to clear them as well.
If COMT activity is already running slower, this can mean one more load added to a pathway that’s already working at a more gradual pace, alongside naturally produced oestrogen and, where relevant, the pill or HRT. On its own, this rarely explains a symptom picture. Why This Isn’t the End of the Conversation
As with MTHFR, carrying a slower or faster COMT variant isn’t a diagnosis, and it isn’t fixed destiny either. Gene expression is shaped by sleep, nutrient status, gut health, hormone balance and overall stress load, the same factors that tend to come up when discussing any health concern. Two people with an identical COMT result can have very different lived experiences, depending on everything else happening in their biology at the time.
What COMT does offer is an explanation.
